Do I need an NPI number?

Answer: You need an NPI if anyone other than your client will ever pay for your services: an insurer, Medicare, Medicaid, the VA, an auto or workers' compensation carrier, or a platform that bills on your behalf. You also need one if your employer lists you as the rendering provider on claims. A strictly cash practice does not need one, but it is free and permanent.

You need an NPI when

A payer, platform or employer asks for it. The common triggers: joining an insurance panel; signing up with Headway, Alma, Grow Therapy, Rula, SonderMind, Zeel or a similar platform; enrolling in Medicare or Medicaid; credentialing with VA Community Care; billing auto injury or workers' compensation claims; writing superbills that clients submit to a health plan, HSA or FSA; prescribing or ordering anything that is e-prescribed; or working at a clinic that lists you on claims as the rendering provider.

You do not need one when

Your practice is private pay with no superbills, you never prescribe or order, and no employer or platform has asked. Many providers in that situation get one anyway, because it is free, takes minutes, and lasts for life.

Which professions

The answer differs by profession. Massage therapists usually need one for the VA and auto injury claims. Counselors, social workers and MFTs need one for every panel and platform. Nurse practitioners, physician assistants and physicians always need one. Registered nurses usually do not. See the full list at NPI by profession.

No, as long as you also never write superbills and never prescribe or order. It is free and permanent, so many cash-pay providers get one in case that changes.

Usually yes. Most payers want the rendering provider's individual NPI on the claim even when the clinic bills under its own Type 2 NPI.
